Identification:Every signer must present a valid, current photo ID. It cannot be expired. All identification must contain a photo, physical description, signature and serial number. California recognizes the following types of identification if issued within the last 5 years:


Driver License
• State Issued ID Card
  Senior ID/ Under 18 ID/ CA ID
• U.S. Passport
• U.S. Military ID Card
• Mexican or Canadian Driver License issued by an authorized agency
• Foreign Passport if stamped by the Immigration and Naturalization Service


Preparing Documents: Please fill in all blank lines on any statement, form, letter, affidavit, Power of Attorney, Advance Health Care Directive or other document that requires signature notarization, – but do not sign or date it. I will complete the notary section (and provide it, if necessary) at the time of notarization. By law, a Notary Public is unable to draft, change, or provide legal advice concerning your document, so if you have any questions please speak with the attorney, company or agency that drafted your document.

Willingness & Knowledge: Every signer must be willing to sign the document with no coercion. The signer must also be aware of what he or she is signing and understand its purpose. A signature cannot be notarized if the signer has any cognitive impairment.

Ability to Communicate: All signers must be able to communicate with the notary in English. The document to be signed can be in any language but the notary section must be in English. Notary certificates, if needed, will be provided upon request.


Live Scan Fingerprinting: Please bring the items listed below to your Live Scan Session: 2 copies of the completed "Request for Live Scan Service" form. The form is given to the applicant by the agency requesting the Live Scan and must be completed by the agency and applicant. We cannot provide the form or assist in filling it out.

A valid government-issued photo ID, such as a driver's license or passport.

Payment for the Live Scan service.

It is your responsibility to verify and/or obtain the correct Request for Live Scan Service form with the required ORI Code, referring Agency Set Address and applicable Mail Code. Please complete and print two copies of your Request for Live Scan form.
